I operate more than one eligible business out of one location under one tax identification number. One of my businesses opened in 2015 and one opened in 2020. How do I apply?

You would use Table 1 because your business began operations on or before Jan. 1, 2019. 

This also applies to food truck operators that added food trucks to the fleet at different times. Food truck operators are considered to have one location (the headquarters) and should use the table that corresponds to the date the business first began making sales with its first food truck.
